Photo, Social Media and Video Release Form Waiver
I, hereby grant Reiki Foundation International d/b/a Reiki Home (the “Organization”) permission to take, use and publish my photograph(s) and/or video pictures and audio comments and/or to use my name in social media (e.g. Facebook, Instagram) I authorize the Organization to use and publish the photos and/or video pictures in any of its publications, brochures, materials, newsletters, social media, website, or in any other manner including those not yet created, without any review or further notice from me.
I understand that such uses may be commercial or noncommercial as defined by law, but that all uses and publication will be for the sole purpose of advancing the tax-exempt purposes of the Organization. Should I change my decision at a future date, I agree it is my sole responsibility to contact the Organization in writing requesting the photograph, social media and/or video be removed. The Organization has thirty (30) days to comply with my request.
Notwithstanding the foregoing, I agree that the Organization may continue to use any previously-printed paper or similar materials including my image after the date of my removal request so long as the Organization does not thereafter print any new copies or otherwise use my image. I hereby waive any right to royalties or any other compensation arising from or related to the use of the photograph.
I hereby agree to release, defend, and hold harmless the Organization and its agents or employees, including any firm publishing and/or distributing the finished product in whole or in part, whether on paper or via electronic media, from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any publication, use, misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reducing or production of the finished product, its publication or distribution.
By signing below, I acknowledge that I have read, understand and agree to every part of this release, that I am at least 18 years old and that I am competent to contract in my own name.
